We did work only for the U.S. Navy, but stupid seems to be universal. And this was an easy root-cause determination. Other failures are far more esoteric – especially when the crumpled remains are not available.

Or even when they are available, with a herd of black-boxes to go with them. I am still awaiting the FAA analysis of the China Eastern MU-5735 crash in March 2022.
