You’ve seen this problem once before, should have put systems in place to EVER allow a repeat but now… Inexcusable! Damn glad they won’t be sealing me up in one of these capsules in January 2019.

“damaged during manufacture”…..and it passed how many quality control inspections and tests? With the knowledge it was to be used on a manned launch? Words fail me.
